Job Description
Our client is seeking a compassionate and highly organized Remote Oncology Nurse Navigator to provide comprehensive support to cancer patients and their families through a fully remote model. In this vital role, you will act as a central point of contact, coordinating care, facilitating communication between patients and healthcare providers, and ensuring patients receive timely access to necessary services and resources. You will empower patients by providing education, emotional support, and assistance in navigating the complexities of their cancer journey.

- Provide direct patient support via phone, video calls, and secure messaging platforms.
- Assist patients in understanding their diagnosis, treatment options, and care plans.
- Coordinate appointments with physicians, oncologists, radiologists, surgeons, and other specialists.
- Facilitate referrals to ancillary services, such as social work, nutrition, physical therapy, and support groups.
- Educate patients and families on managing treatment side effects and symptom management.
- Ensure clear and consistent communication between the patient, family, and the entire healthcare team.
- Advocate for patient needs and help overcome barriers to care.
- Maintain accurate and confidential patient records within the electronic health record (EHR) system.
- Monitor patient progress and adherence to treatment plans, intervening as needed.
- Develop and deliver educational materials on various aspects of cancer care and survivorship.
- Collaborate with the care team to develop individualized care pathways.
- Participate in virtual case conferences and team meetings.
- Stay updated on the latest advancements in oncology treatments and patient care models.
- Registered Nurse (RN) license in good standing.
- Minimum of 3 years of experience in oncology nursing.
- Experience in nurse navigation, care coordination, or patient advocacy is highly preferred.
- Strong understanding of cancer pathophysiology, treatment modalities, and psychosocial aspects of cancer care.
- Excellent communication, interpersonal, and empathetic listening skills.
- Proficiency in using telehealth platforms, EHR systems, and other virtual communication tools.
- Ability to work independently and manage a caseload effectively in a remote setting.
- Strong organizational skills and attention to detail.
- Knowledge of community resources and support services for cancer patients.
- Bachelor of Science in Nursing (BSN) or higher preferred.
- Oncology Nursing Certification (OCN) is a plus.
- Must have a reliable internet connection and a dedicated, private workspace.
